What happens to the penis in a penile fracture?
traumatic rupture of the Corpus cavernosa and tunica albuginea in an erect penis
What is penis fracture caused by?
blunt trauma where the penis if violently deviated away from its axis e.g in intercourse with female on top, forceful masturbation, falling from bed with an erect penis or forceful correction of congenital chord
What are the clinical features of a penile fracture
popping sensation or hearing a snap with immediate pain, swelling and detumescence, swelling and discolouration, deviation towards opposite side of lesion, firm immobile haematoma (rolling sign) if butterfly shaped then urethral injury
What are the differential diagnosis for penile fracture?
rupture of dorsal artery or veins, or rupture of the suspensory ligaments
What are the investigations for a penile fracture?
clinical diagnosis, blood tests as require a repair, cavernosography but may cause complications, ultrasonography retrograde urethography if have urethral injury suspected
What is the management for penile fracture?
analgesia, urgent surgical exporation and repair, circumferential incision and penile skin de gloved, haematoma evacuated and tear is identified and repaired, abstinence for 6-8 weeks
What are the complications of a penile fracture?
penile curvature during erection, penile paraethesia and dyspareunia (painful erection)
