Life cycle

1- adults live in small intestine of dogs and carnivores animal
2- gravid segment/ proglottids pass in their feces
3- they contaminate food / water and it is ingested then by intermediate host ( human - rabbit- cattle - pig - camel - sheep)
4- egg hatches into onchospere penetrates intestinal wall to other organs ( liver - lung - kidney - brain- bone) becoming hydait cyst
5- human is a end blind host
6- dogs are infected by eating meats of its intermediates containg hydait cyst

Structure of hydatid cyst

A

Walls
- inner germinal layer ( endocyst)
- laminated layer ( exocyst)
- outer fibrous layer ( host immune reaction )

Contents
- hydatid fluid : highly immunogenic
- scolics
- brood capsules ( germinal layer containing scolex
- daughter cysts ( laminted layer surrounding brood capsules )

Contents may keep attached to wall or fall forming hydatid sands

- inflammatory reaction ~> encapsulation of cyst ~> mechanical pressure inside organ

  • rupture of the cyst ~> fluid ~> severe allery reaction ~> anaphylactic shock ~> death if significant amout

1- liver ( 66% ) (4) : asymptomatic until complicated
- anaphylactic shock
- liver failure
- biliary obstruction
- jaundice

2- lung ( 22%)(4 ~> 4) :
- pneumothorax
- pleuritis
- lung abscess ( 2 nd bacterial infection)
- rupture in main bronchus
• cough
• dyspnea
• hemoptysis
• rusty sputum with hydatid fluid

3- kidney (2)
- hematouria
- hydatid fluid in urine

4- brain
+ intra cranial pressure

5- bone

Diagnosis

A
  • history : related to dog ?
  • clinical
  • laboratory (6)
  • NO ASPIRATION
  • NO ADULTS /EGG ARE FOUND IN HUMAN
  • ultrasound on infected organs to observe hydatid cyst
  • ELISA
  • CBC : eosinophilia
  • intradermal test of casoni
    Injection of hydatid material in forarm and then observe for 20 min ( immediate hypersensitivity then its positive )

Treatment

A

3
1- drugs : albendazole for 3 months
2- surgical removel of cyst
3- PAIR ( percutunous Aspiration of fluid - INJECTION of formalin ( scolicidal ) Reaspiration )
To prevent anaphylaxis and metastasis ( scolex)
