Type 2 Diabetes Flashcards
What is the other name for type 2 diabetes?
NIDDM
What is the aetiology of type 2 diabetes?
Expanded upper body visceral fat
AND
Aberrant insulin producing genes
What does an expanded upper body visceral fat cause?
Increased FFA’s in the blood
- This reduces insulin receptor sensitivity
Can an incredibly fat person still have normal [BG] levels?
Yes
- Type 2 required aberrant large insulin producing genes too so the pancreas is unable to cope with the reduced sensitivity
In short, what is type 2 diabetes?
Reduced insulin receptor sensitivity COMBINED WITH an inability to produce enough insulin
What are the symptoms of type 2?
Polydipsia
Polyuria
Weight loss
Lethargy
Itching
Poor healing
What diagnostic tests should be done for DM?
Random glucose finger prick
OGTT
Diagnostic HbA1c
What HbA1c levels are diagnostic?
> 48mmol/l
What surgery can be done for fat people?
Bariatric surgery
- Sleeve gastrectomy
- Roux en Y reconstruction
What is first line pharma treatment for Type 2?
Metformin
What is the action of metformin?
Improves insulin action
What is second line treatment of Type 2?
Sulphonylurea
Thiazolidione
What is the action of sulphonylureas?
Increases insulin release
What is the action of Thiazolidione?
Improves insulin action
What are the third line treatments?
DPPIV inhibitors
SGLT2 inhibitors
