Describe the presentation of trigeminal neuralgia
Unilateral maxillary or mandibular div short stabbing pain
Lasts 5-10 seconds
Causes of trigeminal neuralgia
- Idiopathic
- Classical
- Vascular compression of the trigeminal nerve - Secondary
- MS
- Space occupying lesion
Triggers for trigeminal neuralgia
- Cutaneous
- Wind/cold
- Chewing
- Touch
Age affected by trigeminal neuralgia usually
40+
Red flags for trigeminal neuralgia
Younger patients
Sensory deficit in facial region - hearing loss
First line tx for TN
Carbamezipine 100mg 1 tablet 2x daily
Second line
Gabapentin
Side effects of Carbamazepine
Thrombocytopenia
Neutropenia
Electrolyte imbalances
Paraesthesia
Liver toxicity
Surgery indications for TN (2)
- When approaching max tolerable medical management even if pain controlled
- Pts with significant drug use
Surgery options for TN (3)
- Microvascular decompression
- Destructive central procedure
- Stereotactic radiosurgery
Triggers for cluster headache
Alcohol when in bout but not in remission
Features of a Paroxysmal Hemicrania
1-40 attacks daily
Last 2-30mins
Sharp throbbing v severe unilateral pain
Orbital + temporal region

Last 15 mins -3hrs
