PCSK9 inhibitors
Inhibit degradation of LDL receptors
Side effects: Nasopharyngiitis, arthus reaction, joint pains
Effects of important drugs
- Statins: LDL lowering, modest HDL raising and triglycerides lowering effects
- PCSK9 Inhibitors: LDL lowering
- Niacin: LDL lowering, HDL raising, tri lowering
- Fibrates: HDL raising, Tri lowering, modest LDL lowering
- Bile acids: LDL lowering
- Absorption inhibitors: LDL lowering
Statins side effects
- Myalgias
- Memory impairment
- Raising blood sugar
- Can cause an elevation in LFTs which is a marker for hepatotoxicity
Fibrates side effects
- Myopathies especially in patients with reduced creatinine clearence in combination with statins
Niacin side effects
Flushing and hepatotoxicity
When are ace inhibitors and arbs beneficial to use
In a post MI patient that has a reduced ejection fraction
Nitrates
- Headache is a common side effect
- No mortality benefit
- Nitroglycerin, isosorbide nitrate and mononitrate
When do we use fibrinolytics and what is their mechanism of action.
They are used in acute MI ST elevation or in stroke. Their mechanism of action lies in the conversion of plasminogen to plasmin that breaks the cloths.
Examples are Alteplase, reteplase and tenecteplase
When is heaprin used
In stemi and anstemi or when the patient is increased risk of emolization. It is also used to inhibit thrombotic propogation.
It is an anticoagulent, can be used instread of fibrinolytics
What are the proven mortality reducing drugs
- Beta blockers
- ARBs and ACE inhibitors
- Aldosterone antagonists
- Hydralazine/isosorbide dinirtate combination
Beta blockers side effects
Hypotension, bradycardia and fatigue
