What is PTSD?
person exposed to a traumatic event where person experienced/witness actual or threatened death/injury/integrity of self or others
duration of > 1 month
What does a pt experinces with PTSD?
re-experience event
avoid stimuli associated to the event
sx of increased arousal
What negative cognitions are associated with PTSD?
sense of blame of self or others
estrangement from others
diminished interest in activities
inability to remember key aspects of the event
How do you tx PTSD?
SSRIs
Cognitive processing therapy (support groups, EMDR)
*avoid addictive Rx due to inc risk of substance abuse
Acute stress disorder sx are similar to …
PTSD
duration differs though: 3 days - 1 mo post trauma for Acute stress disoder
What is adjustment disorder?
development of emotional/behavioral sx in response to an identifiable stressor
occuring within 3mo of stressor
What are the time frames for:
- PTSD
- Acute Stress Disorder
- Adjustment Disorder
- lasts > 1mo post event
- 3 days - 1 mo post event
- within 3 mo of event; usually doesn’t persist past 6mo
What is seen in adjustment disorder?
significant distress out of proportion to severity of stressor
impairment in function
What behaviors can be seen with adjustment disorder?
depressed mood
anxiety
mixed anxiety and depressed mood
disturbance of conduct
mixed disturbance of emotions and conduct
What are the diagnostic criteria for Conversion Disorder?
- one or more sx or deficit affecting voluntary motor or sensory function
- Psych factors associated with sx or deficit
- Sx or deficit is not intentional
- Sx or deficit cannot be fully explained by a general medical condition, or by substnace, or a cultural behavior
- Sx or deficit causes significant distress or impairment in social, occupational, or other areas
- Sx or deficit not limited to pain or sexual dysfunction, doesn’t occur exclusively during couse of Somatizaiton disorder, and isn’t better accounted for by dif dx
What sx are associated with conversion disorder?
paresthesias and anesthesias
weakness
paralysis
pseudoseizures/psychogenic seizures
involuntary movements
sensory disturbances (blindness, mutism)
How is somatoform disorder tx?
therapeutic relationship
team approach (pain management, neurology, psych)
CBT
Hypnosis
Anti-anxiety medications (clonazepam)
Define dissociative amnesia
inability to recall important, personal info
often related to trauma
What is dissociative fugue?
sudden, unexpected travel away from home
inability to recall one’s past/personal identity
What is dissociative identity disorder?
multiple personality disorder
often survivors of sexual abuse
