Impending Main Gear Box Failure

A

Indications - abnormal noise; yaw kicks, torque fluctuations, unusually high power settings for given flight condition

Main Gear Box Chip Locator Light

A
  1. Instruments and caution lights - CHECK
  2. If malfunction not verified with secondary indications - LAND ASAP. when flying over water, return to the nearest ship or shore at reduced airspeed and altitude. Altitude should be sufficient enough to perform a quick flare if an en route emergency landing is necessary.
  3. If malfunction is verified by secondary indications - PERFORM IMPENDING MAIN GEAR BOX PROCEDURES
    * NOTE - 30 second time delay incorporated into the main lube sump and aux sump chip detector systems. MAIN GB chip detector on the chip locator panel is not affected.
  4. If chip detector light extinguishes, continue mission and monitor all instruments

Main Gear Box Oil System Failure

A

*NOTE - not necessary to abort a flight with MGBAUX LUBE PUMP light. Make sure condition is fixed before next flight.

INDICATION - MGB OIL PRESS light, complete loss of oil pressure indicated.

  1. Airspeed - reduce airspeed to minimum required for flight
  2. Altitude - reduce
  3. Land as soon as possible. Duration of flight not to exceed 30 minutes.

INDICATION - MGB OIL PRESS and MGB AUX LUBE PUMP, oil pressure drops rapidly or decreases below min.

  1. Land immediately
    • CAUTION - loss of oil pressure accompanied by increase in noise, yaw kicks, or power surges indicates seizure is imminent.

INDICATIONS - MGB OIL PRESS, MGB OIL HOT, max or min indications on gauge.

  1. Maintain reduced airspeed and low altitude.
  2. Land as soon as possible
  3. When flying over water return to nearest suitable landing site at reduced airspeed and altitude.

INDICATIONS - if oil pressure changes by 10 psi or drops below 45 psi.

Main Gear Box Oil Cooler Blower Failure

A

Indications - abnormal noise or thump, short duration medium frequency vibes. Main Gear box oil temp and caution lights/gauge indications. Abnormal indications on accompanied systems.

Components susceptible to damage - 1st stage hyds, tail rotor drive shaft, tail rotor control linkage, tail rotor pressure and return lines, no2 engine.

  1. Check appropriate instrument and caution lights
  2. Land as soon as possible

Free-Wheeling Unit Failure

A

Indications - Nf > Nr, torque will drop to zero, slight to moderate left yaw kick.

  1. Collective - maintain Nr at or above 90%
  2. Airspeed - as required for single/dual engine capability
  3. Pickle - as desired
  4. Speed control lever - as required (affected engine to min gov)
    • WARNING - if no2 is at or below min gov and Ng is less than 75%, hot exhaust back flow can cause fire hazard. Limit airspeed to less than 85kias. If FWU slippage has occurred in no2 and dual engine can be maintained consider shutting down the engine. If FWU lock up is suspected, maintain no2 at no less than 75% Ng.
    • CAUTION - no1 or no3 lockup, maintain affected engine at or above ground idle to ensure lubrication
  5. Land as soon as practical
  6. Affected engine (if locked up) - shutdown last

Nose Gear Box Failure

A

Indications - abnormal noise, change in noise frequency, high engine torque (caused by binding or restriction).

  1. Instruments and caution lights - check
  2. Collective - maintain Nr
  3. Airspeed - as required for single/dual engine capability
  4. Pickle - as desired
  5. Speed control levers - as required (shut down affected engine, conditions permitting)
  6. Land as soon as practical

Nose Gear Box Chip Locator Light

A
  1. Instruments and caution lights - check
  2. If malfunction is verified or chip locator light does not extinguish - perform nose gear box failure procedures
  3. If chip locator light extinguishes, continue mission and monitor nose gear box instruments and associated cautions
    • NOTE - an increase in torque for a given flight condition is positive sign of the nose gear box binding or restriction

A

MGB to AGB Drive Shaft Failure

A

Indications - loss of all associated systems and malfunction was not proceeded by loss of oil press, chip lights, abnormal noises.

  1. APP - start
  2. Land as soon as possible
    • NOTE - will result in loss of 2nd stage, U1, U2, engine start pump, AFCS, no1 and no3 generators

AGB Chip Locator Light

A
  1. Instruments and caution lights - check
  2. If malfunction verified or chip locator light does not extinguish - lower landing gear, land as soon as possible
  3. If chip locator light extinguishes - continue mission and monitor closely
    • NOTE - AGB Failure will result in loss of 2nd, U1, U2, AFCS, engine start pump, no1 and no3 gen

IGB or TGB Chip Locator Light or Failure

A

Indications - chip light and oil pressure light. Heading control difficulty, yaw kicks, high frequency vibes, unusual or loud grinding noises

  • chip light combined with any indication above
    1. Land immediately
  • only single caution light with no secondaries
    1. Land as soon as possible
    2. If caution light extinguishes - continue mission and monitor closely
    3. If caution light returns - land as soon as possible
