Scapular Precautions
-no WC propulsion
-pulling or pushing w/ UE
Paraplegia: WC to Supine
-T12
-tripod position; backward if body jacket
-Can use arms fully, might need assistance
-no maintenance of tenodesis
-use sliding board
- Breaks, remove foot rests and arm rests, put on gait belt
- Bring feet together
- Unweight to bring hip up and pull in slide board by protracting and depressing
- Use arms to pull up onto board and mainting trunk with sliding board
- Use wheel to push off while on board
- Slide back bringing legs onto table OR pull legs up w/ strap
- Bring body down with triceps
Automatic Fails
-failure to maintain tenodesis
-failure to maintain body jacket precautions
-Pulling on GH
-Stepping over Pt
-Pt falls
Paraplegia: Supine to WC
- Cross legs
- Use arm momentum to swing into sidelying
- Maintain backwards or forwards tripod and bring legs down
- Repeat WC steps^^
C7 Tetraplegia
-little hand function
-no trunk balance
-protect TENODESIS
-have triceps, serratus and partial lats
C7 Tetraplegia: Supine to WC
- Cross legs
- Use UE momentum to swing into sidelying
- Use back pocket or army crawl method to reach sitting w/ PT supporrting shoulders
- Maintain forwards tripod and bring legs down
- Repeat WC steps ^^ maintaining forwards tripod
Head-Hips relationship
Back Pocket Method
-Bring hands to buttocks
-depress and protract scapula to walk elbows up w/ PT assistance
Army Crawl Method
-Roll to the side
-Assist Pt with brings shoulders under elbows using scap
-crawl toward legs
-Hook legs with one arm and use momentum to sit up into forward tripod
Head-Hips Relationship
-use head to move hips
- count to 3
-head starts in same direction of hips
-Hips going L, head goes up and L to down and R
-Hips going back, head goes up and quickly down
-Hips going forward, head goes down and quickly up
C7 Tetaplegia: WC to Supine
- Pt can hook to remove arm rests and foot rests and sliding board using tenodesis grip
- Use head hips relationship and hands to move on sliding board
- Use hooked arm for momemtum
- Use wheel to push off while on board
- Maintain forward tripod
- Scoot backward and bring head back and quickly down while pushing hips back
- Bring legs up
- Slowly bring body down

C6 Tetraplegia
-no hand
-no trunk balance
-protect TENODESIS
-NO triceps
-Some serratus and pec
C6 Tetraplegia: Supine to WC
- Cross legs
- Use UE momentum to swing into sidelying LOWER than head to prevent hitting face
- PT behind to assist propping on elbows
- Use army crawl method to reach sitting w/ PT supporting shoulders and assist weight shifts.
- Throw arm back using ER
- Sit close enough to maintain forwards tripod and bring legs down
- Scoot forward and bring head down and quickly up while pushing hips forward
- Repeat WC steps ^^ maintaining forwards tripod with locked out elbow
Head-Hips relationship
C6 Tetraplegia: WC to Supine
- remove arm rests and foot rests and sliding board using tenodesis grip
- Pt can hook to help with gait belt donning and forward movement in WC
- Use leg squeeze to put slide board under
- Use head hips relationship and hands to move on sliding board
- Use hooked arm for momemtum AND explain arm ER
- Use wheel/footrests to push off while on board (push with protractors)
- Explain arm ER Maintain forward tripod
- Scoot backward and bring head back and quickly down while pushing hips back (PT hands on outside of arms to stop unlocking elbow)
- Bring legs up
- Get behind Pt to assist with lying down
- Quickly lower one arm at a time by IR shoulder onto elbows

C4/C5 Tetraplegia
-weak shoulder girdle
-no hand
-no trunk balance
-protect TENODESIS if WB
-Weak extensors
-Treat as dependent in early days
-GH shoulder issues
Instruct Pt about care while
C4/C5 Tetraplegia (Dependent): Supine to WC
- Apply gait belt
- Bring Pt to edge of bed, cross legs, cross arms
- Bring legs off the edge
- Bring Pt into sitting and stabilize legs
- Rearrange legs and arms
- Tilt Pt to use slide board
- Transfer
C4/C5 Tetraplegia (Dependent): WC to Supine
- Apply gait belt
- Scoot forward and bring sliding board under butt
- Arrange legs and arms
- Transfer
- Lay Pt down and bring legs
- Remove gait belt
Wheelchair Set Up
Explain EVERYTHING
- Lock chair
- Cross legs to remove rests
- Readjust chair (show pizza slice)
- Safety belt; ask about breathing
- Remove seatbelt
- Explain head hips relationship
