Swing Through

A

-a lot strength for more clearance
-crutches ONLY
-riskiest
-easiest

  1. Shift weight onto AD
  2. Push down using UE protraction
  3. Quickly tuck head to help abs
  4. Bring legs passed AD
  5. Lift head to keep momentum and bring crutches forward

4 Point Reciprocal

A

-most energy efficient
-use hip flexors or quadratus or head hips

  1. Shift weight onto contra crutch
  2. PT guards weight shift
  3. Bring leg forward (hike or flex hip or head-hips) while pushing ipsi UE
  4. Shift weight onto ipsi crutch
  5. PT guards weight shift

Sit to Stand

A

Facing Towards
Sitting Down:
1. Shift weight onto contra and release crutch to grab arm rest
2. Lean onto arm rest and release other arm to grab rest
3. Swivel in chair while bringing arm to contra side of swivel

Standing up:
1. Bring butt at edge of chair
2. Move legs away from roll direct
3. Cross leg in direction of roll on top
4. Bring arm across and other arm behind on arm rest and swivel
5. Hop forward and grab AD

Facing Away
Sitting down:
-Reverse tripod to eccentrically lower down

Standing up:
- w/ head down, quickly look up and push through UE
-catch yourself

Ramp

A

-harder to to, start with drag to with small steps
-PT must block hips more due to inc jacknifing
-crutches or RW

  1. Bring crutches close to with hop space
  2. Bring crutches up and shift weight
  3. Push into UE and tuck head
  4. Bring legs right before crutches and lift head
  5. Use small steps
  6. Come down with small steps

PT must guard hips that tend to go down the ramp

Curbs: Up

A

Forward Up:
1. Bring crutches close to with hop space
2. Bring crutches up and shift weight
3. Push into UE and tuck head
4. Bring legs right before crutches and lift head

Backward Up:
1. Bring crutches behind legs on curb
2. Shift weight onto UE
3. Push into UE and tuck head HARD
4. Bring legs back and lift head

Curbs: Down

A

Forward Down:
1. Bring crutches close to curb but still on curb
2. Leave crutches beind you as you shift weight, tuck head and bring legs down and forward
3. Lift head and quickly bring crutches down

Backward Down (Walker):
1. Bring crutches close to curb but still on curb while facing away
2. Leave crutches in front of you as you shift weight, tuck head and bring legs down and back
3. Lift head and bring crutches down

Doors

A

Hydraulic Doors:
1. Shift weight onto one crutch and use arm to open door wide enough to clear
2. Use crutch to hold door open
3. Swing to or through to clear door

Normal Door:
1. Shift weight onto one crutch and use arm to open door wide enough to clear but NOT too wide to not be able to reach
2. Swing to or through to clear door
3. Reach behind to close door in steps

Fall to Floor and Stand Up

A
  1. Have Pt throw AD out to the side, out of the way, but easily assessed
  2. Have Pt turn head to side and land with controlled shoulders
  3. Pt reach to bring crutches close to their side
  4. Walk out arms and push into downward dog
  5. Grab one AD and push/WB onto one side
  6. Grab the other and stand up
