thyroid patho pots Flashcards
1
Q
Gross Graves Disease (3)
A
- Beefy red appearance - increased vascular flow
- diffused symmetrical enlargement
- no distinct nodules
2
Q
Histo Graves Disease (3)
A
1 .follicles are large and irregular, show infoldings
2. Follicles have pseudopapillary hyperplastic infoldings - no true fibrovascular cores
3. scalloping of colloid - resorption by follicular cells
3
Q
Gross Hashimotos Thyroiditis (4)
A
- diffusely enlarged and firm
- pale tan cut surface, homogenous appearance
- no discrete nodule
- no haemorrhage or necrosis
4
Q
Histo Hashimotos Thyroiditis (2)
A
- lymphoid follicles with germinal centers
- atrophic thyroid follicles - hurthle cell changes (oncocytic) granular cytoplasm
5
Q
Gross Multinodular Goitre (6)
A
- multinodular enlargement
- cut surface has multiple variable nodules
- brownish spongy appearance
- cystic degeneration
- foci of haemorrhage
- areas of fibrosis
6
Q
Histo MNG
A
- colloid-rich follicles lined by crowded columnar epi, giving rise to pseudopapillae
- no fibrous capsule around mass - distinguish from follicular adenomas and carcinomas
7
Q
Gross Follicular Thyroid Adenoma (4)
A
- well encapsulated, circumscribed single nodule
- cut surface is brown and homogenous
- focal haemorrhage
- no necrosis or cystic change or other nodules
8
Q
Histo Follicular Thyroid Adenoma (5)
A
- no capsule invasion
- no vascular invasion
- follicles containing colloid of varying sizes (mixed micro and macro)
- nuclei of follicular cells is rounded and euchromatic
- rest of thyroid is normal
9
Q
Gross Follicular Thyroid Carcinoma (4)
A
- non-encapsulated mass
- invasion into neighbouring structures
- tan fleshy cut surface
- foci of necrosis and haemorrhage
10
Q
Histo Follicular Thyroid Carcinoma (4)
A
- capsular invasion
- vascular invasion
- within the tumour, microfollicles containing small amount of colloid
- nuclei are small, round and uniform with speckled chromatin
11
Q
Gross Papillary Thyroid Carcinoma (3)
A
- multinodular neoplasms
- non-encapsulated and invasive growth with irregular borders
- cut surface is pale tan (look for papillary structures)
12
Q
Histo Papillary Thyroid Carcinoma (5)
A
- Ground Glass nuclei/ Orphan Annie eye nuclei - from diffused chromatin
- Cytoplasmic Inclusions in nuclei
- Nuclear Grooves (coffee bean nucleus)
- maybe psammoma bodies in vasculature - calcification
- papillary structure with fibrovascular cores lined by these atypical follicular cells
13
Q
Gross medullary thyroid Carcinoma (4)
A
- Multiple pale nodules
- Irregular borders, non-encapsulated with invasive borders
- pale tan cut surface
- necrosis or haemorrhage
14
Q
Histo medullary Thyroid Carcinoma (4)
A
- islands/ nests of malignant cells
- nuclei with salt and pepper chromatin
- extracellular pink amyloid material
- Congo red stain shows pink amyloid deposit on light microscopy
