Thyroid Bump Pathology Flashcards
what are the three main changes seen in histo of multinodular goiter?
hyperplasia
colloid accumulation
nodule formation
what does colloid accumulation look like in multinodular goiter?
lots of colloid within one follicle
what does hyperplasia look like in multinodular goiter?
hypercellular area
how do follicular adenomas appear grossly?
they are well circumscribed and clearly a nodule
what are the histo findings of a follicular adenoma?
encapsulated area of hypercellularity, with normal thyroid outside of capsule
how do you differentiate between a follicular thyroid carcinoma and follicular adenoma?
follicular thyroid carcinoma will have either blood vessel or capsular invasion
what is the classic cell seen in follicular carcinoma?
hurthle cells..very light pink
what is main histo change to note in papillary thyroid carcinoma?
CYTOLOGiC appearance changes
what are the cytologic nuclear changes seen in papillary thyoid carcinoma?
crowded cells with nuclei dispersed and irregular nuclear contour
what is irregular about nuclear contour in papillary thyroid carcinoma?
nuclear grooves and intranuclear pseudoinclusions
what thyroid cancer has psammoma bodies?
papillary thyroid carcinoma
what can papillary thyroid carcinoma sometimes lead to in thyroid?
cystic space
how does medullary thyroid cancer appear on histo?
lots of packets of uniform oval blue cells and spindle shaped cells…all in C cell area
what three thyroid neoplasms are easily picked out on FNA?
papillary thyroid carcinoma
medullary thyroid carcinoma
anaplastic thyroid carcinoma
what two thyroid neoplasms are hard to decipher on FNA?
follicular adenoma and carcinoma
