what is thrombocytopenia

Immune Thrombocytopenic Purpura
Thrombotic Thrombocytopenic Purpura.
pathophiosoligy of ITP
autoimmune condition
where antibodies (IgG) from the
spleen target the platelets.
who does ITP affect
Usually affects children after a viral
infection.
Adults can get it too after
chronic disease
pathophyisology of TTP
a condition where there is an
issue (inherited or autoimmune)
with a protein called ADAMTS13. It is
responsible for inactivating VWF so
when it is not doing its job, it results in multiple clots and reduces platelet
levels
who does TTP affect
Usually in adults with chronic
diseases such as HIV, malignancy or
autoimmune conditions
what do ITP and TTP have in common
They both have purpuric rashes
which is more unwell out of ITP and TTP
ITP - more systemically
TTP - more unwell.
signs and symptoms of ITP
Purpuric rash, easy bleeds,
menorrhagia
signs and symptoms of TTP
Purpuric rash, menorrhagia,
microangiopathic haemolytic
anaemia, renal insufficiency,
fatigue, fever, neurological
symptoms
diagnosis for ITP
- Thrombocytopenia,
- Increased Megakaryoblasts on blood film
diagnosis for TTP
- Thrombocytopenia
- schistocytes on blood film
(fragmented RBCs), - decreased ADAMTS13
treatment for ITP
- Watch and wait,
- Prednisolone
- IV IgG
treatment for TTP
- Plasma exchange
- Prednisolone
- Rituximab (monoclonal antibody that
targets B cells
