Therapist drift
Failure to deliver treatments that we have been trained to deliver, or failure to deliver them adequately, even where resources exist to allow us to do so.
Aka therapy-interfering behaviour by therapists
Costs
Patients problems are maintained/worsened
Increased negative attitutde to therapy
Increased hopelessness about recovery
Increased feeling of incompetence and decreased satisfaction for the therapist
Wasted treatment resources
General cause
Therapist falsely believe they have to protect the patient form demands of therapy, avoiding distress. Often in reaction to immediate larger or smaller crisis in therapy.
Three elements
Cognitions:
- Fundamental attribution bias
- Schemas
- Black or white thinking
- Blinded by own thoughts
Emotions:
- anxiety bout own performance/harming patient
- Fear of negative evaluation
- Guilt
Behavior:
- Safety behavior, faling to push for change
- working too hard, not doing what you should do
