explain housing and adaptations – role of the LA
-Adaptations could be in either the individuals own home, or an already adapted house
-assessed by the local authority, social services to enable individuals to live independently
I.e. a stairlift

-LA have too ensure they involve individuals in the decisions regarding their care
-This means giving people a choice of where and how they want to live
-Must ensure homes are accessible and designed with individuals needs in mind
- LA must meet housing needs

-If people want to remain at home social services must do their best to make this happen
-They can do this by giving special equipment, and accommodation
-If you can’t adapt the house the other options are residential homes or sheltered accommodation

It’s a legal document which set out a young persons needs in education, health and social care
documents what they can and can’t do and what has to be done to meet these needs
Outlines any special educational needs a child has, and the provision that must be put in place to help them

What is the key to commissioning?
working in equal partnership with individuals who use the services, their families and communities and organisations

Services can be bought by other organisations for example, beds in a hospital
This gives better choice of services available
Individual must be involved from start to finish
promoting greater range of care – decentralisation and commissioning
Before personalisation local authorities would not source care
If the care was not available for individual, would just miss out
Now because things are outsourced the individual can choose a service that best meet their needs
How local authorities might not support personalisation
officers may not be trained in a person centred approach
They may not be a range of services available locally to meet wants and needs
Insufficient funding for services
Budgets allocated may be insufficient for meeting care needs
