who has review meetings and how often ?
individual who have care plans, support plans or EHCPS will have review meetings at least once a year
what is the purpose of review meetings?
- ensure care is effected or to improve care
- generate actions
- meeting changing needs to the individual
- review personal budget - ensure it’s sufficient to meet care needs
what is the importance of review meetings?
- to generate actions - to rectify issues in care plan, ensures goals are met
- building and sharing information collaboratively - collaborative approach
- putting the individual at the centre of the meeting - ensures their desires and wishes are being met
what is a facilitator?
the facilitator supports the person whose review it is and makes them the centre of the meeting. They are a neutral person.
what two things does a facilitator do in a review meeting?
1) they lead questions, focusing on the individual
e.g. what is working/ not working
2) They help the group compare and discuss to agree a common direction. The individual must be consulted to see if they agree
how can the individuals be made to feel as comfortable as problems during the meeting?
1) family/ friends allowed at meeting
2) refreshments
3) individual picks time/ location of meeting (gives them choice and control)
4) ensure individual is at centre
how will an individuals key worker help them prepare before the meeting?
1) asking individual who they’d like to attended the meeting
