When to notify BOP of change of ownership
Must notify the Board of Pharmacy within 30 days
Information that must be included:
● Full name of new owner
● Attached official bill of sale
● If being transferred/purchased by a corporation the application must include
o Copy of corporation’s articles of organized (signed by secretary of MA)
o Copy of corporation’s foreign corporation certificate (if out of state)
o Statement of each officer/director’s name, address and position
o D/b/a (doing business as)
o Privately owned – name and address of stockholders with stock type and amount
● If MOR will also change, must follow directions below in addition
Time to Notify Board of Changes to PIC?
Must notify the board within 10 days
Information that must be included:
● CS (C2-5) inventory signed by both outgoing MOR and incoming MOR (If outgoing MOR is unable (due to death, serious illness or termination of license for inappropriately handling Controlled Substances) then a staff RPh may be authorized to sign off on inventory if BOP was notified
● The pharmacy permit and certificate of fitness (if applicable)
Required fees ($525.00)

Must notify the board in advance of address change
A new application must be submitted to the Board and all previously issued permits must be returned (Permit to operate, CS registration and certificate of fitness)
