- gender of the patient
- Individual history including allergies and drug reactions
- Comprehensive list of medications and relevant devices
- Pharmacist comments
how long must patient records be kept?
12 months from the last date of entry
who can request patient records
A Power of Attorney (if proof available)
Parents/guardians (if patient <18)
Pharmacists must make a reasonable effort to obtain the following for a patients profile
- Name
- address
- phone number
- DOB
- gender
- Allergies, drug reaction
- Comprehensive list of medications, and relevant devices
- Any additional comments related to medication usage
What Must RPh Review on Patient Profile Prior to Dispensing
● Overutilization or under-utilization
● Therapeutic duplication
● Drug-disease contraindication
● Drug-drug interaction
● Incorrect drug dosage or duration of drug treatment
● Drug-allergy interactions
● Clinical abuse or misuse
● Any significant change in drug, dose or directions
When are pharmacists REQUIRED to check the PDMP
● Must use prior to each time a narcotic** Schedule II-III or a **benzodiazepine is issued
