Neurotoxin produced by C.tetani
Tetanospasmin
Maternal tetanus
Occuring during pregnancy or within 6 weeks after
C.tetani
Anaerobic, gram-positive, spore-forming rod
Minimum lethal human dose of toxin
2.5 ng/kg
MC infection sites of tetanus in adults
Superficial abrasions to the limbs
Clincial manifestations occur
Once toxin reached PRESYNAPTIC inhibitory nerves
Affected first in generalized tetanus
face and jaw
MC initial symptom
Trismus
Autonomic disturbances occur
2nd week of severe tetanus: Inc CV mortality risk
Labile BP Tachy Brady Heart block GI stasis Sweating Inc tracheal secretions Acute renal failure
Treatment for tetanus
Metronidazole
Alt: Penicillin
Given early to deactivate any circulating tetanus toxin and prevent its uptake into the nervous system
Antitoxin (HTIG or equine antitoxin)
Recovery from teranus
4-6 weeks
