What is a hydrocele?
accumulation of fluid within the tunica vaginalis
features of a hydrocele
- attatched to testes / not seperate
- transilluminate
- possible to feel above the lump
Mx of a hydrocele
- first line - surgery
2. second line - aspiration
what is a varicocele?
- dilatation of veins in the pampiniform plexus within the spermatic chord
- 95% on the left hand side as there is increased venous pressure when left testicular vein –> left renal vein
what are the features of a varicocele?
- feels like ‘bag of worms’
- disappears lying down
- comp - may cause subfertility
Mx of varicocele
conservative reassurance
if complications consider surgery
what is an epididymal cyst?
transilluminable swellings felt in the scrotum behind the testes
How does a hernia present in the groin?
- indirect inguinal hernia can present has a swelling in the groin
- cough impulse
- not possible to feel above it
what is Epididymitis?
- infection of the epididymis leading to scrotal
- caused by chlamydial infection in young men
- may be caused by ascending urehtra and bladder infection
How does epididymitis present?
- scrotal discomfort
- pain during micturition
- fever
- features of underlying sti/uti
mx of the epididymitis?
- if STI - 2 week course of doxycyline
2. analgesia
How does testicular torsion present?
- sudden onset severe testicular pain
- abdominal pain
- vomitting
Mx of testicular torsion
urgent surgical intervention and urgent orchidopexy - testes sutured into the tunica vaginalis to prevent further torsion
Name 3 differential diagnosis of a painful testicular lump?
- torsion
- epididymitis
- strangulated hernia
What are the signs seen in testicular torsion?
- swollen and tender
- high riding
- cremasteric reflex absent
- Phren’s sign is negative
what is ‘phren’s sign’?
- helps differnetiate testicular torsion vs epididymitis
2. physical lifting of the testes relieves the pain of epididymitis but not the pain of testicular torsion
Ix of testicular torsion
- bloods - inc inflammatory markers
- urine dipstick and MSSU
- Doppler ultrasound scan to confirm diagnosis
Ix of epididymitis?
- Bloods - inflammaotry markers
- MSSU
- urethral swabs / urine chlamydia
- US of scrotum
differentiating torsion vs infection
Torsion
- more acute and sudden pain
- urine dipstick
- no infective symptoms
- lifting does not ease
Infection
- gradual onset
- more infective symptoms (fever)
- positive urine dipstick
- lifting may ease
