What is BPH?
benign hyperplasia of the prostate
what are some voiding symptoms?
- weak flow
- straining
- terminal dribbling
- incomplete emptying
(HITS)
what are some storage symptoms?
- frequency
- urgency
- nocturia
What are some complications of BPH?
- UTI
- acute urinary retention
- chronic renal failure (hydronephrosis) due to chronic renal failure
first line mx
- alpha antagonist - Tamusolin / Alfuzosin
- decreases smooth muscle tone
adverse side effects of alpha antagonists?
- dizziness
- postural hypertension
- dry mouth
2nd line mx
5 alpha reductase inhibitors - Finasteride
- block conversion of testosterone to DHT which is known to induce BPH
- reduces prostate volume - slows down disease progression
adverse side effects of 5 alpha reductase inhibitors?
- erectile dysfunction, reduced libido and ejaculation problems
what is the gold standard mx?
TURP
transurethral resection of the prostate tissue
when is long term catheterisation indicated?
patients with prostatic obstruction unsuitable for TURP
Ix and examinations
- Examinations
- DRE/abdo/genitals - Ix
- PSA/USS/eGFR
- Flow rate and residual volume
- urinalysis / check for uti
acute urinary retention
- urological emergency - sudden inability to pass urine
- severe lower abdominal pain
- palpable bladder and dull to percuss
Name 3 complication of TURP?
- haemorrhaging
- septicaemia
- retrograde ejaculation
- incontinence
Mx of acute urinary retention?
- urgent urinary catheterisation
