What are risk factors for testicular cancer?
Age 20-45, Caucasian, cryptorchidism, previous testicular cancer, HIV, FH
What are the types of testicular cancer?
Germ cell tumours (seminoma, non-seminomatous germ cell tumour, mixed germ cell tumour), Non-germ cell tumours (lymphoma)
What is the most common form of testicular cancer?
Seminoma
What is a typical presentation?
A painless non-transilluminable testis lump, haemospermia
What is the first line investigation?
Urgent ultrasound of testes
What tumour markers might you look for?
Alpha-fetoprotein, beta-hCG, LDH
What are the main differential diagnoses?
Testicular torsion, scrotal hernia, hydrocele
What surgical option is there?
Urgent radical inguinal orchidectomy
What are some common complications?
Treatment related nausea, treatment related neutropenia, infertility
What is Stage I?
No evidence of metastasis
What is Stage II?
Infradiaphragmatic node involvement
What is Stage III?
Supradiaphragmatic node involvement
What is Stage IV?
Lung involvement
How would you manage metastatic disease?
Retroperitoneal lymph node dissection, chemo, radiotherapy
