age above 45, caucasian, previous testicular cancer, cryptorchidism and HIV

seminoma (55%)
teratoma (33%)
mixed seminoma teratoma (12%)

presentation of testicular cancer
A
painless, hard scrotal bump
gynaecomastia (tumour produces hCG)
lymphadenopathy

investigations for testicular cancer
A
bloods; FBC, U&Es, LFTs, tumour markers (alpha fetoprotein, B-hCG, LDH)
urine pregnancy test; positive shows tumour
CXR- metastases, testicular US

management
A
radical orchidectomy, radiotherapy and chemotherapy
