A
  • Size (may not exceed __ or cannot be greater than __ of the material thickness)
  • Distribution: single can be a stress riser
  • Shape
  • Location
  • Evidence of abnormal service: not used as designed
  • Evidence of long service: wearing out
  • Lack of documentation: Provide details on when and what the specs were when made, what is aloud/beyond repair
  • Reporting and disposition - Required for a report to be valid, what to do after inspection (fix it, remove it, out of service etc.)
