Reliever:
- What is in this type of inhaler?

Explanation:

  • What is it used to relieve?
  • How does it work?
  • “You shouldn’t need this more than ‘X’ times a week if your asthma is well controlled.” - What is X?
A

SABA/LABA - e.g. salbutamol

This is useful to help relieve sudden asthma attacks.

It works by relaxing the airways so that you can breathe more easily.

3 times a wk

Ask your GP for a review if you are using this more frequently. I would like you to inhale (x puff(s)) when you feel short of breath.”

Steroid/ICS - beclometasone

You use this inhaler to reduce the risk of having asthma attacks.

It helps to reduce the swelling in the airways and stops them from tightening up.

Adherence - they shouldn’t miss doses

Demonstration - Preparation:

pMDI is the main one used in OSCE’s. What does it stand for?

(1) Device test (if not used for more than 5 days):
- What should be done to test the device? - very self-explanitory

(2) What do some pMDI’s have on the side or bottom?
(3) What should be checked to make sure it is still in date?

A

Pressurised metered-dose inhaler

(1) Remove cap and shake well
(2) Point the mouthpiece away from you and press the canister to release a puff into the air.

(1) Remove inhaler cap and shake well

They reduce the amount of drug deposits in the mouth.

Spacer Devices - Procedure:

(1) Remove inhaler cap and shake well
(2) Attach the mouthpiece to the space device
(3) Breathe out gently - away from the device

Children <3 yrs old:

  • What should be used to help form a seal?
  • How many degrees should the spacer be put upwards to?

Single breathe technique:
(4) How long do they hold their breath for??????**

(5) Breathe in again without pressing canister to make sure you have taken in all the air.

Multiple breath technique:

(1) Start breathing in and out normally
- If using AeroChamber, what can be used to see when they are breathing?
- How many breaths should an older child do?
- How many seconds should a very young child be kept using it?

(2) Once breathing pattern established, depress canister.
- How many more tidal breaths should be done?

Why should it be left to drip dry instead of wiping dry?

A

Wiping dry can cause static within the device and drug particles will stick to the side of the spacer.
