Different types of inhaler:
Reliever:
- What is in this type of inhaler?

- “You shouldn’t need this more than ‘X’ times a week if your asthma is well controlled.” - What is X?
SABA/LABA - e.g. salbutamol
This is useful to help relieve sudden asthma attacks.
It works by relaxing the airways so that you can breathe more easily.

Steroid/ICS - beclometasone
You use this inhaler to reduce the risk of having asthma attacks.
It helps to reduce the swelling in the airways and stops them from tightening up.
Adherence - they shouldn’t miss doses

SMART (Single maintenance and Reliever Therapy):
- Give an example of this?
- Why is this better than having 2 inhalers, 1 as a reliever and the other being a preventer?
Symbicort
It is both a preventer and reliever.

Rinsing their mouth after use to prevent oral candidiasis
Canestan (fluconazole) is used if it does develop.
Demonstration - Preparation:
pMDI is the main one used in OSCE’s. What does it stand for?
(1) Device test (if not used for more than 5 days):
- What should be done to test the device? - very self-explanitory
(2) What do some pMDI’s have on the side or bottom?
(3) What should be checked to make sure it is still in date?

Demonstration - Procedure:
pMDI is the main one used in OSCE’s.
(1) Remove inhaler cap and shake well
(2) Breath ___ gently
(3) Place mouthpiece in mouth start breathing ____, then _______, and continue deep ______.

Spacer Devices:
Why are they good for those using steroid inhalers?
Who are they used for?

They reduce the amount of drug deposits in the mouth.
<8 yr old, or older but having difficulty with MDI
Volumatic and AeroChamber - look up
Spacer Devices - Procedure:
(1) Remove inhaler cap and shake well
(2) Attach the mouthpiece to the space device
(3) Breathe out gently - away from the device
Children <3 yrs old:
- What should be used to help form a seal?
- How many degrees should the spacer be put upwards to?
Single breathe technique:
(4) How long do they hold their breath for??????**
(5) Breathe in again without pressing canister to make sure you have taken in all the air.

(1) Start breathing in and out normally

(2) Once breathing pattern established, depress canister.
- How many more tidal breaths should be done?

What should be done if another dose is needed?
Remove and shake inhaler - wait 30 seconds
Spacer Devices - Care instructions:
Why should it be left to drip dry instead of wiping dry?
Wiping dry can cause static within the device and drug particles will stick to the side of the spacer.

- How many puffs, 30 seconds apart should be used before calling 999 in an asthma attack?
