TCM Internal Medicine Test #3 Flashcards
Ischemic Stroke
TIA is a temporary interruption of the blood flow to an area of the brain - a warning that a stroke may follow. A clot in an artery, a drop in blood pressure, or a change in heart rhythm or rate may all reduce blood flow to the brain and result in a TIA. Symptoms include vision problems, difficulty understanding words, difficulty speaking, unusual behaviors and thought processes,
loss of conciousness, seizures, dizziness, vertigo, trouble walking, weakness or numbness on one side of the body. Symptoms will go away after 10-20 min or beocme a full stroke.
MERIDIAN STROKE IN TCM
Hemorrhagic Stroke
Hemorrhagic stroke: intracerebral hemorrhage, SAH (subarachnoid hemorrhage). An artery in the brain leaks or bursts.Due to abnormalities of the blood vessels, high blood pressure, protein deposits along the blood vessels, traumatic brain injury. Blood irritates the brain tissues calling swelling which will destroy brain tissue. Subarachnoid hemorrhage is bleeding in the area between the brain. Caused by bleeding from an arteriovenous malformation, bleeding disorder, bleeding from a cerebral aneurysm, head injury, unknown cause, use of blood thinners, injury/trauma.
ZANG FU STROKE IN TCM
Two types of strokes in TCM
Closure and Collapse
Two types of closure strokes in TCM
Yin and Yang
Two types of meridian stroke in TCM
Excessive and deficient
Types of excessive stroke in TCM
- Liver yang
- Phlegm fire
- Wind phlegm
Stroke Location
Location: Brain (related to HT, LV, SP, KD)
Stroke Nature
Nature: excessive in symptoms, deficiency in root, upper excess and lower deficient.
Stroke pathological factors
Pathological factors: WIND, FIRE, PHLEGM
Stroke duration
Duration: acute stage (2 weeks to 1 month), recovery stage (2 weeks/1 momth to 6 months), sequela (more than 6 months)
Stroke key point of pathogenesis
Key point of pathogenesis: disorder of qi and blood, rebelling upward to attack the brain. Deficiency (qi), fire (liver and heart), phlegm (wind phlegm and phlegm damp), wind (liver wind), qi (rebellious qi), blood (blood stagnation).
Stroke Root
Root: liver, kidney yin deficiency or blood deiciency
Stroke symptoms
Main symptoms: sudden collapse, loss of consciousness, hemiplegia, slurred speech or speechless, deviation of the tongue and mouth (facial paralysis), numbness in half side of the body.
Other symptoms: headache, dizziness, vomiting, urinary and fecal incontinence, or urinary and fecal obstruction, irritability, cramping, profuse phlegm, hiccup.
Meridian stroke
Meridian stroke: 4 major symptoms: hemiplegia, deviation of the tongue and mouth (facial paralysis), slurred speech, numbness in half side of the body
Zang Fu Stroke
Zang Fu stroke: 5 major symptoms: loss of consciousness, hemiplegia, deviation of the tongue and mouth (facial paralysis), speechless, numbness in half side of the body
Difference between Meridian stroke and Facial Paralysis
MERIDIAN STROKE
1. Decreased consciousness or alertness, hemiplegia
2. Still able to close the eye on the affected side, wrinkle in the forehead
Blood clot blocks a blood vessel in the brain
Meridian stroke: no loss of consciousness
FACIAL PARALYSIS
1. NO decreased consciousness/alertness, hemiplegia
2. NOT able to close the eye on the affected side, as well as wrinkle in the forehead
Facial nerve becomes inflamed (peripheral facial paralysis, bell’s palsy)
Zang Fu stroke: sudden coma
Stroke pathomechanism
REBELLIOUS QI AND BLOOD OBSTRUCTION OF WIND, FIRE PHLEGM, STASIS \+ TRIGGER FACTOR CAUSES OBSTRUCTION OF BRAIN VESSEL OR BLOOD LEAKING OUT OF VESSEL
Symptoms: 4 major symptoms (hemiplegia, deviation of the tongue and mouth, slurred speech, numbness in half side of the body), dizziness, distention headache, eyes and face red, irritability, easy to get angry, bitter taste in the mouth, dry throat, constipation, yellow urine, red or crimson tongue, yellow or dry yellow coating, wiry or rapid pulse.
