Test of cognitive functioning post TBI
Levels I-VIII with least to most functional
RLA Level I
aka No Response
Deep sleep, unresponsive to any stimuli
RLA Level II
aka Generalized Response
Patient reacts non-purposefully and nonspecifically to stimuli
Responses may be physiological changes, gross body movements, and/or vocalization
RLA Level III
aka Localized Response
Pt reacts specifically but inconsistently to stimuli
May follow simple commands (close eyes, squeeze hand) in an inconsistent and delayed manner
RLA Level IV
aka Confused-Agitated
Bizarre and non-purposeful behavior
Unable to cooperate directly with treatment efforts
Incoherent and/or inappropriate vocalizations
General and selective attention impaired
Lacks short- and long-term recall
RLA Level V
aka Confused-Inappropriate
Able to respond to simple commands fairly consistently
More complex commands = non-purposeful/random responses
Gross attention to environment but highly destractible
Short periods of conversation
Severely impaired memory
Able to perform learned tasks with structure
RLA Level VI
aka Confused-Appropriate Shows goal-directed behavior Follow simple directions consistently Carryover for learned tasks Responses may be incorrect d/t memory problems, but are appropriate to the situation
RLA Level VII
aka Automatic-Appropriate
Appropriate and oriented in hospital/home setting
Automatic, robot-like routine behavior
Minimal-no confusion with shallow recall of activities
Carryover with new tasks but at slower rate
Initiate social or recreational activities with structure
Judgement impaired
RLA Level VIII
aka Purposeful-Appropriate
Aware and repsonsive to environment
Integrate past and present events
Carryover for new activities with no need for supervision once learned
Glascow Coma Scale
Neuro assessment tool after TBI to determine arousal and cerebral cortex function
Lowest possible score on Glascow Coma Scale
3; the lowest you can score in each of the three sections is a 1
Glascow Coma Scale score correlating to severe brain injury
8 or less
GCS score indicating moderate brain injury
8-12
GCS indicating mild brain injury
13-15
Highest possible score on GCS
15; Eye opening graded 1-4, Best Motor Response graded 1-6, and Verbal response graded 1-5
