what is CIWA?
- a tool that commonly helps clinicians assess for alcohol withdrawal
- admitted patients may begin to withdrawal a few days after admission which can lead to serious complications
what are the 10 things CIWA measures that are associated with withdrawal?
- nausea and vomiting
- tremor
- paroxysmal sweats
- anxiety
- agitation
- tactile disturbances / hallucinations
- auditory disturbances / hallucinations
- visual disturbances / hallucinations
- headache
- orientation and clouding of sensorium
what is the total CIWA score used to determine?
whether benzodiazepines should be given to ameliorate symptoms or avoid seizures
what should be taken along with CIWA scoring?
vital signs
when will alcohol withdrawal symptoms start?
- depends on usage/ amount
- 8-12 hours after last drink
- sometimes can be ok for a couple of days, then experiance withdrawl
- always ask when last drink was
what is basic protocol?
diazepam 20 mg PO q1-2hr until symptoms abate
what should be given to a pt if they have history of withdrawal seizures?
Diazepam 20mg q1h for a min of 3 doses
what should be given to a pt if they have severe liver disease, severe asthma or respiratory failure?
lorazepam SL, PO 1-2mg tid-qid
what should be given to a pt who is experiencing hallucinations?
haloperidol 2-5mg IM/PO q1-4 hrs (max 5/day)
what women are at the top of the list for detox and rehab?
pregnant women with substance abuse issues
