Maladaptive pattern of substance abuse defined as 3 or more of the following signs in 1 year:
- Tolerance - need more to achieve the same effect
- Withdrawal
- Persistent desire or unsuccessful attempts to cut down
- Significant energy spent obtaining, using or recovering from substance
- Important social, occupational, or recreational activities reduced because of substance abuse
- Continued use in spite of knowing problems it causes
Substance dependence
Maladaptive pattern leading to clinically significant impairment or distress:
- Recurrent use resulting in failure to fulfill major obligations at work, school, or home
- Recurrent use in physically hazardous situation
- Recurrent substance-related legal problems
- Continued use in spite of persistent problems caused by use

Emotional lability, slurred speech, ataxia, coma, blackouts
Alcohol intoxication
Serum GGT is a sensitive indicator of

Lab AST is twice ALT
Alcohol use
Autonomic hyperactivity and delirum tremens
Severe Alcohol Withdrawal (use benzos for DT)
Euphoria, respiratory and CNS depression, decreased gag reflex, pupillary constriction (pinpoint pupils), seizures (OD).
Opioid intoxication
Treatment: Naloxone, Naltrexone
Sweating, dilated pupils, piloerection (“cold turkey”), fever, rhinorrhea, yawning, nausea, stomach cramps, diarrhea (“flu-like symptoms”)
Opioid withdrawal
Treatment: Methadone, Buprenorphine (partial agonist)
Low safety margin, marked respiratory depression
Barbiturate intoxication
Treatment: Assist respiration, BP
