Types of gastritis and how they are caused
Erosive (NSAIDs, ETOH, stress)
Non-erosive (H pylori, CMV, candida)
Most sensitive diagnostic test for gastritis?
Endoscopy with biopsy
Treatment of gastritis
- H2 blocker or PPI
- If H Pylori, then triple or quadruple therapy with PPI+abx+immunosuppressant
Define peptic ulcer disease
- Erosion in gastric or duodenal mucosa
- Greater than 5 mm through muscularis
What type of ulcers are MC in 55-70 yo?
Gastric
Which type of ulcers are MC?
Duodenal
Which type of ulcers are MC in younger patients?
Duodenal
Causes of peptic ulcer disease
- NSAIDs
- H pylori
- Smoking
Pain with gastric ulcer
Pain occurs with eating
Pain with duodenal ulcer
Pain is relieved by eating but returns 2-4 hours postprandial
Treatment of peptic ulcer disease
- PPI preferred
- Gastric 8 weeks tx
- Duodenal 4 weeks tx
- H pylori 14 day combo therapy
Treatment of GERD based on severity
- H2 blockers for mild
- PPI for mod-severe
Define Barrett esophagus
Normal esophageal epithelium replaced with columnar
Who does Barrett’s esophagus affect?
Up to 10% GERD patients
Define gastroparesis
Decreased or absent gastric motility WITHOUT mechanical lesion
