Define Generalised Convulsive Status Epileptics

A

Generalised seizures causing a loss of consciousness and effecting the entire brain. This is a result of abnormal or excessive neuron activity.

Acitvity >5min or multiple seizures without COMPLETE regain of consciousness between seizures.

Continuous Generalised Seizure Management

A
  • Protect patient from injury
  • OP/NP airway
  • Oxygen via NR mask or assist ventilations if required
  • Administer Midazolam IM
  • Repeat dose once only after 10/60 if required
  • Consider MICA

Midazolam Doses

A

Newborn (1 day old) = 0.5mg
Small/large Infant (< 12 months) = 1mg
Small Child (1-4 years) = 2.5mg
Medium Child (5-11 years) = 2.5-5mg

  • Administer High Flow 02
  • Repeat Initial dose ONCE ONLY after 10/60 if seizure continues

Define Febrile Convulsions

A

A generalised seizure in children occurring typically between 6 months and 5 years. Usually associated with infection (viral/respiratory) and high temperature.

  • Sudden temp spike over 39 degrees
  • Due to an immature hypothalamus
  • Not related to epilepsy although seizure can look the same
  • Seizure is generally self limiting otherwise treat with Midaz
