Rivermead Behavioral Memory Test
★ Designed to assess memory abilities of adults, detect and identify moderate severe memory impairments, and monitor change in memory following a brain injury

Mini-Mental State Examination (MMSE)

★ Addresses areas of cognition including:

Canadian Occupational Performance Measure (COPM)
★ Semi-structure interview-based rating scale
★ Designed to detect change in a client’s self-perception of occupational performance over time
★ Fosters collaboration between client and therapist
★ Used as an initial assessment and as an outcome measure
Occupational Questionnaire (OQ)
★ Self-report or interview-based log and rating scale
★ Designed to collect information regarding client’s use of time and how it relates to client volition
★ Logs perceptions of competence, value, and enjoyment of occupations
Model of Human Occupation Screening Tool (MOHOST)
★ Rating form based on observation, discussion, and record review
★ Screening tool to determine the need for OT services and document funding
★ Generates goals and outcomes, specifically related to changes in participation
Occupational Performance History Interview II (OPHI-II)
★ 3-part assessment including a semi-structured interview, rating scale, and life history narrative
★ Rates client identity, occupational competence, and impact of the environment
★ Clients must respond to in-depth interviews
