Signs of Systemic illness (SIRS Criteria)
- Temp >38 or <36
- HR >90
- RR > 24
- WBC >12 or <4
Etiological Agent of Purulent SSTI
Staphylococcus Aurues
Mild SSTI
No systemic symptoms
Moderate SSTI
Systemic Signs of Infection but hemodynamically stable
Severe SSTI
Systemic Signs of Infection +:
- Immunocompromised
- Hemodynamically Instable
- Failed I&D and PO antibiotics
Mild Purulent SSTI Treatment
I &D –> No antibiotics
Moderate Purulent SSTI Empiric treatment
Bactrim or Doxycycline
Moderate Purulent SSTI De-escalation
Oral: [DACC]
D: Dicloxacillin
A: Augmentin
C: Cephalexin
C: Clindamycin
Severe Purulent SSTI Empiric Therapy
IV: –> Cover MRSA
Vancomycin –> DOC
Linezolid –> Second Choice
Daptomycin
Telavancin
Ceftaroline
Severe Purulent SSTI De-escalation
