- Redness

What are the secondary findings of a diabetic foot infection?
- Non-purulent secretions
- Discolored granulation tissue
- Foul odor
What are the MRSA and Psuedomonas Risk factors?
MRSA= Previous Hx, High local prevalence, severe infection
Pseudomonas = Warm climate, water exposure, high local prevalence
What are the etiological agents of DFIs?
1.) Aerobic + Cocci: Staphylococcus or Streptococcus
2.) Aerobic - Cocci: P. aeruginosa
3.) Anaerobes
Mild Diabetic Foot Infection Criteria
0.5-2cm Erythema
Moderate Diabetic Foot Infection critiera
2 + cm Erythema or involving structures deeper than skin.
Severe Diabetic Foot infection criteria
Local infection w/ 2 SIRS criteria.
Mild DFI empiric treatment
Doxycycline OR Bactrim
Mild DFI De-escalation
Cephalexin OR Augmentin OR Clindamycin

+=Augmentin
Anaerobe=Clindamycin
Mild DFI Treatment Duration
1-2 weeks.
Moderate-Severe DFI Empiric Therapy
- Vancomycin + Piperacillin
- Vancomycin + Cefepime + Metronidazole
- Vancomycin + meropenem
Moderate-Severe DFI De-Escalation
Ampicillin/Sulbactam
Cefoxitin
Ceftriaxone + Metronidazole
Ciprofloxacin + Clindamycin
Moxifloxacin
Ertupenem
Moderate DFI treatment duration
1-3 weeks.
Severe DFI treatment duration
2-4 weeks.
