Abdomen:
A
- Tenderness.
- Rigidity ==> Pt will contact his abdominal muscles when you try to palpate the abdomen.
- Rebound tenderness.
- CVA tenderness.

Chest:
A
- SOB.
- Wheezing ==> whistling.
- Decreased resp sounds.
- Increased fremitus ==> Pt will say 99 in a coarse voice.

Nervous system:
A
- Confusion.
- Dementia.
- Extensor plantar response (Babinski).
- Absent or hyperactive tendon reflexes (stroke, DM).
- Tremor (resting, intentional).
- Facial paralysis.
- Hemiparesis.
- Gait abnl.
- Ataxia — chorea.
- Hearing loss.
- Tinel — Phalen.
- Nuchal rigidity — Kernig — Brudzinski.

Eyes:
A
- Visual loss (central, peripheral). In a young pt, this may be due to MS.
- Photophobia ==> Dim the light!
- Lig lag.
- Nustagmus.

Muscles and joints:
A
- Muscle weakness.
- Rigidity.
- Spasticity.
- Parkinsonism ==> Shuffling gait (difficulty initiating and stopping ambulation, small steps, no swinging of the arms), resting tremor, masked facies, rare blinking, cogwheel rigidity.
- Restricted ROM of joints.

Bruits and murmurs:
A
- Renal artery stenosis.
- Thyroid bruit.
- Carotid bruit — hush hush.
- Heart murmur — hush hush.

Skin:
A
Artificial skin discoloration (eg painful red spots on the shin for erythema nodosum in a patient with sarcoidosis).

Real physical exam findings:
A
- C-section, appendectomy, cholecystectomy, or other scares.
- Real nevus (mole). Ask the pt and adivse him to check it routinely.
- Skin lesions such as pytiriasis rosea, seborrheic dermatitis, or acne vulgaris.
- Real heart murmur.
- Pt w/ sore throat may present with enlarged tonsils.
