Q

Sample closure:

A
  1. Mr. Green, you may have a fractured bone, a simple sprain, or a dislocation of the shoulder joint.
  2. We will need to obtain an x-ray of your shoulder and arm to make a diagnosis, and more precisely imaging studies such as an MRI may be necessary as well.
  3. Your safety is my primary concern, and I am here to offer you help and support whenever you need it.
  4. Sometimes living with a family can be stressful for the whole household.
  5. Have you considered moving to an assisted-living community or to an apartment complex for seniors? If you are interested, I can arrange a meeting with our social worker, who can assess your social situation and help you find the resources you need.
