LoC — Hx:
A
- Preceding symptoms (nausea, diaphoresis, palpitations, pallor, lightheadedness).
- Context ==> Exertional—postural—traumatic—stressful—painful—claustrophobic experience—dehydration.
- Associated tongue biting—incontinence—tonic-clonic movements—prolonged confusion—dyspnea or pulmonary embolism risk factors.
- H/o heart disease—arrhythmia—HTN—DM—alcohol or drug use.

LoC — PEx:
A
- VS + orthostatics.
- Complete neuro.
- Carotid and cardiac exam.
- Lung exam.
- Exam of the lower extremities.

Numbness/weakness — Hx:
A
- Distribution ==> unilateral—bilateral—proximal—distal.
- Duration—progression.
- Pain (especially headache, neck or back pain).
- Constitutional Sx.
- Other neuro sx.
- H/o DM—alcoholism—Atherosclerotic vascular disease.

Numbness/weakness — PEx:
A
- VS.
- Neurologic and musculoskeletal exams.
- Relevant vascular exam.

Fatigue and sleepiness — Hx:
A
- Sleep hygiene.
- Snoring.
- Waking up choking/gasping.
- Witnessed apnea.
- Overexertion—stress—depression—other emotional problems.
- Lifestyle changes—shift changes at work.
- Diet and weight changes.
- Other constitutional symptoms—symptoms of thyroid disease.
- Hx of bleeding or anemia.
- Medications—alcohol—caffeine—drug use.

Fatigue and sleepiness — PEx:
A
- ENT exam ==> conjunctival pallor, oropharynx/palate, lymphadenopathy, thyroid exam.
- Heart, lung, abdominal, neurologic, and extremity (pallor, coolness at distal extremities).
- Consider rectal exam and occult blood testing.

Night sweats — Hx:
A
- Patterns ==> escalating—waxing—waning.
- Precipitants ==> food—medications.
