End of life Care discussion pointers

  1. Silences, exploring thoughts
  2. Check understanding
  3. Collect all thoughts, concerns, fears, anxieties and expectations.
  4. Be curious. Explore any cues they may have hinted.
  5. Understand the relative; are they well? Their mood, their responsibilities as a carer, other stressors in their life, the impact of the situation.
  6. Summarise the ICE. A powerful way to help the relative feel understood and also focuses the management plan.
  7. Empathise. The immensity of the situation and its potential impact on the person.
  8. Enquire about communication between the relatives and patient.
  9. Provide options/suggestions to manage each concern in turn.
  10. Consider if other professionals or support groups may be helpful.
  11. Empower with regard to their concerns.
  12. Plan future meetings. Is anything required in the meantime?
  13. Safety net: rescue treatment and contact numbers in the event of sudden deterioration.
