Should you Remove an Inpaled Object?
No
DO NOT know path the object has taken, cannot determine the underlying damage. If removed further damage may occurs. Any tempo made effect on bleeding caused by the object may be lost.
Describe Primary, Secondary, Teritary blast injuries.
Primary: usually due to sudden change in environmental pressure, mostly affecting gas filled organs as they are stretched beyond their capacity.
Secondary: flying debris acing as high velocity missiles
Tertiary: due bystander being propelled through the air by blast and impacting with other objects e.g ground, side of building
Three organs most effected by blast injury, Why?
Lungs: gas filled and rapid change environmental pressure distorts the structures = massive damage ( blood, fluid, air embolism may accumulate in damaged tissue.
Bowel: gas filled organ may be perforated with rapid change in pressure = haemorrhage or infection
Ears: eustation tube can only cope with sml changes in pressure. Pressure of blast wave disrupts eustation tube and perforate tympanic membrane.
General Management Soft Tissue Injuries
A- Ensure airway maintained. Consider C-Spine
B- 02, ventilate if required
C- Control major haemorrhage, IVC, IVF, Elevation limb, Cardiac Monitoring
D- GCS monitor, BSL, Pupils
E- Full Exposure to identify any other injuries which may be life-threatening
Pain RELIFE as required, ongoing monitoring vital signs, Sp02, ECG
Specific Management of Avulsion Injury
> Clean around wound with sterile saline
Gently fold the Avulsion into original position
Control Haemorrhage with bulky dressing and direct pressure
Specific Management of Amputation
> Control haemorrhage with direct pressure
Wrap amputated part in gauze and wet with sterile saline
Place it into water proof container or bag and place on ice
Cover stump in sterile dressing and cover with dry dressing
