What is sjogren’s syndrome
an autoimmune disease affecting the salivary glands.
it is mediated by antiobodies
Sicca syndrome
where there is no pathology evident in the salivary glands
dry eyes and dry mouth
Primary sjogren’s syndrome
does not involve the connective tissue
Secondary sjogren’s syndrome
Involves connective tissue - RA, scelorsis, lupus
Things to show positive sign of sjogren’s
- ocular symptoms
- oral symptoms
- ocular signs
- histopathology
- salivary gland dysfunction
- autoantibodies
What autoantiboides are present in sjogren’s syndrome

Ocular symptoms
Daily feeling of dry eyes > 3months
recurrent feeling of grit or sand in eyes
Tear substituites used >3 times a day
Oral symptoms
Daily feeling of dry mouth >3months
Recurrent swelling of salivary glands
freq drinking of liquid to aid swallowing
Ocular signs
scheimer test - <5mm wetting in 5mins
Salivary gland dysfunction
<1.5ml in 15ml of unstimulated saliva flow
Histopathology
looking for lymphatic infiltrate and inflammtory cells
may be destruction of acini
What does it mean if lyphocyte drainage is confined?
Then indiciates not lymphoma
Complications of sjogrens
Recurrent oral infections
Lack of protective factors leading to caries
Function - speech and mastication
Retention of dentures
Increased risk of lymphoma
Enlargment of major salivary glands
Gradual loss of salivary/lacrimal gland due to inflammatory destruction
Management of sjogren’s
diet and OHI
5000ppm tp
salivary stimulant - pilocarpine
pastilles to stimular saliva flow
saliva subs
Treatment for fatigue/joint pain
Hydroxychoroquine - anti malarial drug for tiredness and joint pain
