Shoulder Ortho Special Testing Flashcards
ROM Shoulder Flexion
180 degrees
ROM Shoulder Extension
60 degrees (1/3)
Degrees of shoulder ABD
170 - 180 (move arm up from side)
Degrees of shoulder ADD
50 - 75 degrees (move arm behind shoulder)
Degrees Internal shoulder rotation
60 - 100 degrees (90 deg. bent elbow moving arm toward midline)
Degrees External shoulder rotation
80 - 90 degrees (90 deg bent elbow moving arm toward side
Apley’s scratch test is used for
Pt tries to touch opposite shoulder, test to see limited motion
Apley’s test (motion)
- extension - medial rotation - ADDuction
* flexion - lateral rotation - ABDuction
Internal rotators
Subscapularis
External rotators
Infraspinatus & Teres minor
Abduction
Supraspinatus (ABD for the first 20 degrees)
NEER Impingement Test motion
Passive ABDuction of arm 180 degrees to ceiling, w/internal rotation (palm out when arm up)
NEER Impingement Test is positive when
Pain at top of movement - palm has to be out
IF arm is externally rotated (palm in, toward head) and moved toward ceiling creates pain, check _________
AC joint, this is not a positive NEER
Hawkins-Kennedy Impingement Test
motion
Horizontal arm flexion to 90 degrees, bent elbow to 90 degrees, then medial rotation.
Pain at end of ROM
Positive Hawkins-Kennedy Impingement means
Rotator Cuff Impingement
Positive NEER Impingement means
Rotator Cuff Impingement
What tests are attempting to catch supraspinatus tendon insertion against the coracoid process?
- Neer
* Hawkins-Kennedy
Labral Tears? What test is used to diagnose?
O’Brien
O’Brien Test
Horizontal arm flexion to 90 degrees, #straight arm, horizontal ADDuction (move arm toward middle) 10-15 degrees
a) Pronation (thumb down + pressure applied)
b) Supination (palm up + pressure applied)
When is the O’Brien Test positive?
Pronation - thumb down
What is the diagnosis of a positive O’Brien Test?
SLAP lesion
What should be R/O with the O’Brien Test?
AC joint involvmt, biceps tendonitis
What test is used for Acromioclavicular injury?
Passive Cross-Arm test
Passive Cross Arm test (motion)
Horizontal 90 degree, elbow 90 degrees, rotated toward middle, arm motion horizontal, medial ADDuction
Positive Passive Cross Arm test
Medial, horizontal ADDuction creates pain at AC joint
Speed’s Test (motion)
Break-test (where?)
Forward flexing a supinated arm, while adding downward pressure to the forearm.
Break-test @ 90 degrees horizontal abduction
Positive Speed’s Test
Pain presents on Supination could mean
• biceps tendonitis
• SLAP lesion differentiate with O’Briens
Empty Can Test (motion)
Horizontal 90 degrees flexion, 30-45 degrees horizontal ABDuction, thumb pointing down
Break test
Positive Empty Can Test
- retinopathy of supraspinatus
- supraspinatus tendinosis
- neuropathy of supra scapular nerve
For biceps tendinosis test with
Speed’s
For SLAP lesion test with
Speed’s but differentiate with O’Briens
For supraspinatus tendonosis test with
Empty Can
For neuropathy of supra scapular nerve test with
Empty Can
For rotator cuff impingement test with
- Neer
* Hawkins-Kennedy
AC joint injury test with
Passive Cross Arm test
