Shoulder Imaging Flashcards
What modality is best if an SC jt is dislocated ?
CT
How are the parts of the clavicle described
Medial, middle and lateral rather than proximal and distal
What’s three key signs to look for, that shows us there is proper orientation of a shoulder x-ray
- Entire clavicle is visualised
- Slight overlap of humeral head and glenoid
3 no foreshortening of the scapula body
What view is useful post or during suspected shoulder dislocation ?
AP oblique
How to know alignment of a Y lateral shoulder view is aligned and orientated (3)
- Thin body of the scap should be seen and not superimposed on ribs
- Acromion and coracoid processes should appear as a “Y”
- Humeral head should be superimposed centrally over the “Y” shape
What landmark do we look for to decide on the direction of a shoulder dislocation
Coracoid
What is a hill sach’s lesion?
Denting of humeral head
What is a globe sign?
Posterior shoulder dislocation - looks like a light bulb
Alignment of clavicle x-rays (2)
- Should sit above the gh jt
- Slight overlap with glenoid
What are the indications for fixation of clavicle fractures? (4)
Lateral clavicle fractures
Medial clavicle - maybe
Shortened middle third
Neuro vascular compromise - tenting
How much space is normal between acromion and lateral end of clavicle, for ac joint views
5-8mm
How much height should be seen between clavicle and coracoid process
Roughly the vertical height of the clavicle
What is a type 5 AC jt injury
Greater than 100% height increase in the clavicle
Rockwood classification 5 will be defined as what on an X-ray?
More than two vertical clavicle heights
Rockwood classification 3 will be defined as what on an X-ray?
Up to 2 clavicle heights
