shoulder and elbow flexed with minimal abduction and internal rotation

inferior and medial to the coracoid process
myotendinous junction of muscle belly

pt supine
adduct arm across chest, nearly rolling them over, with inferomedial traction to shoulder
Anterior Acromioclavicular Joint TP
at anterior aspect of AC joint
Ligamentous
Anterior Acromioclavicular Joint Tx
Flexion with internal rotation and traction straight across body
same with anterior deltoid
Infraspinatus TP
Upper: just inferior to spine of scapula, usually more lateral
Lower: Halfway down, and about 2 cm lateral, to the medial border
Infraspinatus Tx
pt supine with ph seated on tx side and hand monitoring under pt.
Upper: flexion of shoulder and elbow (half statue of liberty)
Lower: flexion of shoulder above head (statue of liberty)
note: they may require IR
Teres Minor TP
Posterior head of the humerus or the lateral border of the scapula (under the arm)
Teres Minor Tx
humerus at about 30° of flexion
slight abduction and MARKED ER

ligamentous but can treat posterior deltoid similarly

pt prone and ph standing on contralateral side
pull arm back into extension and hand across to about the midline
Levator Scapulae TP
Superiomedial aspect of the scapula
note: levator elevates the scapula
Levator Scapulae Tx
PT lateral recumbent with affected side up
cephalad hand monitoring
pt head raised (pillow or table) with affected arm behind back to push scapula medial
caudad hand threaded through pt arm and pushing scapula superiorly
Teres Major TP
Posterior surface of scapula just lateral to the inferior angle
Note: Add Ex and medially rotates humerus
Teres Major Tx
pt seated and ph standing behind
“Twist the major’s arm behind his back.”
this Ad Ex and Mr humerus
can grasp at elbow and introduce medial and cephalad force to fine tune
Supraspinatus TP
in the fossa above the spine of the scapula
note: Stabilizes shoulder joint; initiates abduction of the humerus.

pt supine with ph seated on affected side
“45/ 45/ 45”
flexion/abduction/marked external rotation
Subscapularis TP
Anterolateral border of the scapula
under armpit near posterior
pressing from an anterolateral to posteromedial direction
Subscapularis Tx
pt supine with ph seated on affected side extend arm (downwards) with marked internal rotation use slight abduction
aim for “4” or “8” O’Clock position with arm
Latissimus Dorsi TP
about 2-4 cm down humerus
medial of center on anterior aspect
Note: Extends, adducts, & medially (internally) rotates the humerus at the shoulder
Latissimus Dorsi Tx
pt supine with ph seated on affected side extend arm (downwards) with marked internal rotation use slight abduction AND traction
- Extension
- Marked internal rotation
- Slight abduction
- Mild traction
- Same as SUB (subscapularis) but with traction
