Shoulder Complex Injuries Flashcards
Sternoclavicular Joint Dislocation
SC JOINT DISLOCATION (STERNUM & CLAVICLE)
Anterior=indirect trauma , Posterior=direct trauma
Sign:
1=pain, tender
2=sublux, deformity, swelling, pain
3=complete clavicle displacement
Clavicle Fracture
BROKEN/FRACTURED COLLAR BONE
Sign: pain+, tender, selling, dropping outer fragment, loss of function
Care: pain relief, brace 4-6 weeks, move arm below 90deg
Acromioclavicular Joint Separation
AC JOINT SEPARATION Sign: 1= small AC tear, no instability, pain on palpation 2= complete AC tear, small CC tear 3= complete AC/CC tear 4-6= surgical
Anterior Shoulder Dislocation
DISLOCATED TO FRONT
Sign: arm externally rotated/abducted, restricted ROM, altered contour, +apprehension test, swelling
Inferior Shoulder Dislocation
DISLOCATED DOWN
Sign: arm in excessive abduction, soft tissue injury
Posterior Shoulder Dislocation
DISLOCATED TO BACK
Sign: arm in flexion/adduction, possible humeral fractures
Born Loose/Subluxing Shoulder
FREQUENT SHOULDER DISLOCATIONS
Sign: joint laxity, frequent subluxations, dead arm
Rotator Cuff Strain/Tear (under 25)
SHOULDER MUSCLE/TENDON INJURY (decreased stabilization)
Sign: sudden onset after overhead load, twinge in shoulder, some limited function, +drop arm test
Rotator Cuff Strain/Tear (over 40)
SHOULDER MUSCLE/TENDON INJURY
Sign: overhead pain, slow onset, uncomfortable sleep
Shoulder Impingement
PINCHED SUPRASPINATUS TENDOn, REDUCED SUB-ACROMIAL SPACE/BURSA
Sign: overhead pain, diffused pain (acromion/deltoid), difficulty sleeping, relief -90deg, +hawkins-kennedy and +neer impingement test
Care: palliate pain, strength scapular stabilizers/RC
