Shoulder, Breast, and Pectoral Region Flashcards
Pectoral Fascia
Covers the pectoralis major
Axillary fascia
Continuation of pectoral fascia laterally
Clavipectoral fascia
Deep to pectoralis major; encloses subclavius and pectoralis minor
Pectoralis major
Large, fan shaped muscle covering most of the anterior thoracic wall
Origin of Pectoralis major
Sternal half of clavicle, sternum to 7th rib, cartilages of true ribs, aponeurosis of external oblique muscle
Insertion of Pectoralis major
Lateral lip of intertubercular sulcus of humerus
Action of Pectoralis major
Flexes and adducts arm, rotates arm medially
Innervation of Pectoralis major
Medial and lateral pectoral nerves
Supply of Pectoralis major
Pectoral branch of thoraco‐acromial artery, perforating branches of internal thoracic artery
Origin of Pectoralis minor
Outer surface of upper margin of ribs 3–5
Insertion of Pectoralis minor
Coracoid process of scapula
Action of Pectoralis minor
Lowers lateral angle of scapula and protracts scapula
Innervation of Pectoralis minor
Medial pectoral nerve
Supply of Pectoralis minor
Pectoral branch of thoraco‐acromial and intercostal lateral thoracic arteries
Origin of Deltoid
Lateral third of anterior clavicle, lateral acromion, inferior edge of spine of scapula
Insertion of Deltoid
Deltoid tuberosity of humerus
Action of Deltoid
Rotates arm, abducts humerus from 15 to 90 degrees
Innervation of Deltoid
Axillary nerve
Supply of Deltoid
Posterior circumflex humeral artery, deltoid branch of thoraco‐acromial artery
Rotator Cuff Muscles
S i t S Supraspinatus Infraspinatus Teres minor Subscapularis
Origin of the Supraspinatus
Supraspinous fossa of scapula and deep fascia
Insertion of the Supraspinatus
Greater tubercle of the humerus
Action of the Supraspinatus
Initiates abduction of the arm with the first 15 degrees
Innervation of the Supraspinatus
Suprascapular nerve
Supply of the Supraspinatus
Suprascapular artery
Origin of the Infraspinatus
Infraspinous fossa of scapula and deep fascia
Insertion of the Infraspinatus
Greater tubercle of the humerus
Action of the Infraspinatus
Lateral rotation of arm (with teres minor)
Supply of the Infraspinatus
Suprascapular artery
Innervation of the Infraspinatus
Suprascapular nerve
Origin of Teres major
Posterior surface of inferior angle of scapula
Insertion of Teres major
Medial lip of intertubercular sulcus of humerus
Action of Teres major
Adducts and medially rotates arm
Innervation of Teres major
Lower subscapular nerve
Supply of Teres major
Circumflex scapular artery
Origin of Subscapularis
Subscapular fossa
Insertion of Subscapularis
Lesser tubercle of humerus
Action of Subscapularis
Medially rotates arm at shoulder and adducts it, helps hold humeral head in glenoid cavity
Innervation of Subscapularis
Upper and lower subscapular nerves
Supply of Subscapularis
Subscapular artery, lateral thoracic artery
Origin of Teres minor
Upper 2/3 of posterior surface of lateral border of scapula
Insertion of Teres minor
Inferior facet of greater tubercle of humerus
Action of Teres minor
Laterally rotates arm
Innervation of Teres minor
Axillary nerve
Supply of Teres minor
Circumflex scapular artery
Subclavian artery branches
VITamin C and sometimes D
Axillary artery branches
Screw The Lawyer, Save A Patient
First part of the axillary artery
Medial to the pectoralis minor; Superior thoracic artery
Second part of the axillary artery
Under the pectoralis minor; Thoracoacromial trunk and Lateral thoracic arteries
Third part of the axillary artery
Lateral to the pectoralis minor; Subscapular, Anterior and Posterior circumflex humeral arteries
The dorsal scapular nerve innervates…
The levator scapulae and the rhomboid major and minor
The suprascapular nerve innervates…
The supraspinatus and the infraspinatus muscle
The long thoracic nerve innervates…
The serratus anterior
The axillary nerve innervates…
The deltoid and the teres minor
The upper subscapular nerve innervates…
The subscapularis
The lower subscapular nerve innervates…
The subscapularis and the teres major
The thoracodorsal nerve innervates…
The latissimus dorsi
The medial pectoral nerve innervates…
The pectoralis major and the pectoralis minor
The lateral pectoral nerve innervates…
The pectoralis major
The breast extends…
Vertically, from the 2nd to the 6th rib
The breast contains…
1 lactiferous duct per lobe and 15-20 tubuloalveolar glands (with each forming a lobe)
Suspensory ligaments (of Cooper)
Attach the lobes to the skin and divide the glandular tissue into lobes
Arterial supply to the breast
Internal thoracic artery (internal mammary, subclavian), the pectoral branch of thoracoacromial trunk (axillary) and the lateral thoracic artery (axillary)
Lymphatics of the breast
Many nodes drain the breast, but the majority of the nodes drain towards the axilla. Remember CLASP (central, lateral, apical, subscapular, and pectoral nodes)
Where does the highest frequency of breast cancer occur?
In the superior lateral quadrant of the breast, which drains to the axillary nodes.
