Lateral third of clavicle (anterior head), acromion (middle head), and spine of the scapula (posterior head)

Axillary n.
Mostly C5; also C6
Deltoid actions
Abduction after first 15 degrees
Anterior head flexes and medially rotates the humerus
Posterior head extends and laterally rotates the humerus
Subscapularis origin and insertion
Subscapular fossa of scapula
Lesser tubercle of the humerus
Subscapularis innervation and spinal levels
Upper and lower subscapular nn.
Mostly C6; also C5, C7
Subscapularis action
Medial rotation of the humerus

Supraspinatus actions
Initiates abduction of the humerus at the glenohumeral joint, laterally rotates the humerus when decelerating the arm in a throwing motion
