List the standard procedure prior to enclosed space entry?

A

Toolbox Talk.
Allocation of duties to those with correct knowledge and training.
Ventilating the space to be entered.
Cordoning off the area for example if a tank on deck level has to be opened.
Testing the atmosphere with properly calibrated equipment.
Providing adequate lighting.
BA set on standby.
Communications tested.
Rescue plan in place for in the event of an emergency.
Completing a tisk assesment/permit in accordance with SMS
