Sherpath Ch 36: Mobility Flashcards
what helps nurses analyzes activity cues by determining the relationship between information/observations gathered
-determining relationship between cues: physical observation and medical records
-organize and link cues: Categories for connecting/grouping could be related to type of mobility problem, body system affected, unexpected findings from the assessment (especially common alterations in mobility), findings from diagnostic tests, or a combination of these
-consider relevance of cues
what does mobility hypotheses include?
include Impaired Mobility, Paralysis, Fall, and Activity Intolerance. These hypotheses can be actual or “risk for” problems, such as Fall or Risk for Fall
impaired mobility
-Any type of musculoskeletal injury or surgery
-Inability to ambulate independently
-Limited range of motion
-Fractures
-Contractures
-Amputations
-Cause: Any physical alterations that compromise mobility
paralysis
Quadriplegic
Paraplegic
Hemiplegic
Inability to move
Loss of sensation
-Cause: Inability to move related to alterations in neurologic and/or muscular functioning
fall or risk for fall (if a fall has not yet occurred)
-Altered mobility
-Fell at home
-Weakness
-Light-headedness
-Cerebellum injury/damage
-Inner ear injury/infection
-Orthostatic hypotension
-Fall risk scores indicate high risk for falls
-Reduced bone density
-Cause: Inability to maintain a stable position due to skeletal, muscular, or neurologic alterations
activity intolerance
-Dyspnea on exertion
-Oxygen saturation reading below 90% during activity
-Pulse rate increases to above 100 beats/min with activity
-Compromised ability to complete tasks
-Hard to complete activities of daily living
Cause: Inability to perform tasks related to compromised musculoskeletal, cardiac, and/or lung functioning
Other ICNP diagnosis/hypothesis
-Risk for Deep Vein Thrombosis
-Weakness
-Impaired Skin Integrity or Risk for Impaired Skin Integrity
-Anorexia Deficient Food Intake or Risk for Deficient Food Intake
-Constipation or Risk for Constipation
-Social Isolation or Risk for Social Isolation
what is the SMART method
specific, measurable, assignable, relevant, time-based
-expected outcomes from mobility hypotheses
