-Discuss it with them and educate them on their risks/the importance of preventative screenings
-Prescribe a topical estrogen if they are on testosterone therapy to help with comfort prior to the exam (will not interfere with testosterone)
-Allow them to guide the steps of speculum insertion or do it themselves
-Allow them permission to stop the examine at any time
-Make the space a safe space for them

Yes, hormonal replacement or lack there of can change the results. A Transman on no hormones could have a scale based on a man even though it may correlate better with that of the female scale based on their chromosomal gender
