Normal sexual response
Sexual function depends on 2 complex and delicate neurologic pathways
A connection between the brain and genitals, and a reflex arc between the genitals and spinal cord, CNS PNS and ANS involvement
sex dysfunction
involves difficulty with 1 or more aspect of sexual response cycle, bio, psych, interpersonal causes
Primary sex dysfunction- lifelong,
Secondary- after an interval when function has been normal
Must cause significant distress to the individual
Not better explained by a nonsexual mental disorder or as a consequence of severe relationship distress etc
Excitement, plataeu, orgasm, resolution
Excitement- penile ercetion, female clitoral erection, labial swelling, Vaginal transudate, tent, increased sympathetic, nipple erection
Orgasm
Female sexual response- emotional
Serotonin is negative feedback of sex
disorders of sexual desire
Hypoactive- reduced or absent fantasies of desires, up to 20% of pop, more common in women
Sexual aversion- aversion to all sexual contact
sexual arousal excitement disorders
Erectile disorder: 1 or more symptoms, erection is weak or absent, primary vs secondary
Female orgasm disorder
lots of women never have an orgasm
treatment of sexual dysfunction
do not assume anything
SSRI delay orgasm,
Flibanserin- inhibits serotonin neg libido effects
Opioid antagonists and vasodilators
Sildenafil (pde5- viagra)
