Orgasmic disorder is characterized by
Marked delay, marked infrequency, in-orgasmic – lasting > 6 months
Cannot be r/t other physical, mental, or relationship causes
4 types of orgasmic disorder
situational, generalized, primary, secondary
situational orgasmic disorder
Able to achieve orgasm under some conditions, but not others
generalized orgasmic disorder
Unable to achieve orgasm at all
primary orgasmic disorder
Never been able to have orgasm
secondary orgasmic disorder
Previously able to achieve orgasm, now unable
Dyspareunia
pain in labia, vagina or pelvis during or after sex
Must have 1 or more of these:
Difficulty with vaginal penetration during intercourse
Vulvovaginal or pelvic pain during intercourse or attempted penetration
Fear or anxiety about pain before, during, or after vaginal penetration
Pelvic floor muscle tensing when vaginal penetration is attempted.
when is a diagnosis of dyspareunia made?
symptoms are greater than 6 months, cant be explained by any other physical or mental condition
lichens sclerosus, lichens planus, lichen simplex chronicus
Dermatological causes of dyspareunia
Vestibulitis
vulvodynia (nonspecific vulvar pain)
s/s of vestibulitis/vulvondynia
pain at vaginal introitus
unable to achieve penetration d/t pain
pain with tampon attempt
tap vestibule (tissue surrounding vaginal opening) with damp cotton swap
sharp burning pain often at 6 oclock area
